Community Health Needs Assessment
The Community Health Needs Assessment (CHNA) is designed to identify opportunities to improve and promote the health and well-being of residents throughout Green County and the surrounding communities.
Through this process, Jane Todd Crawford Hospital gains valuable insight into the health needs, concerns, and priorities of the population we serve. The assessment brings together information from a variety of sources and community partners involved in healthcare and related services.
The final CHNA report examines key health concerns within the community, identifies available resources and strengths, and highlights challenges that may affect access to care and overall health outcomes. These findings help guide future planning, programs, partnerships, and initiatives aimed at improving community health.
