Social Services

The Social Services Department at Jane Todd Crawford Hospital provides support and guidance to patients and their families throughout the healthcare experience.

Our team works closely with patients, families, physicians, nurses, and other members of the healthcare team to help coordinate care, plan for discharge, and connect patients with appropriate services and resources.

Services may include:

• Insurance pre-certification and authorization assistance

• Discharge planning

• Utilization review

• Patient activities and support

• Swing Bed Program coordination

• Information and assistance with Living Wills and advance directives

Our goal is to help make transitions in care as smooth as possible while ensuring patients and families understand available resources and next steps.
